Air Crash Investigation - American 965
Project Overview
American 965 was on a normal approach to Cali International Airport at night time on December 20th, 1995. After being requested to change runways, the Captain erroneously deleted numerous waypoints from the flight management system. With an increased workload and not familiar with surrounding terrain. The plane turned off course and descended below a neighboring mountain. When commanded to turn back towards the runway the plane turned towards and crashed into the mountainous terrain.

57 year old American male with American Airlines. Experienced with South American operations. Has trouble directing the plane with a relatively new flight management system.
“Ok, understood. Cleared direct to Cali VOR. Uh, report Tulua and altitude 15, that’s 15,000, 3002. Is that all correct sir?”
“Ladies and gentlemen, this is Captain Tafuri, we have begun our descent for landing in Cali. It’s a lovely evening as we had expected. We’ll pass a shower or two on the way in but, uh, at the field right now its, uh, good visibility, temperature of 23 Celsius."
39 year old American male with American Airlines. Not so experienced flying to South America which has an impact on the accident sequence. Becomes quite overwhelmed relatively quickly.
“Well let’s see. We got 136 miles to the VOR, and 32,000 feet to lose, and slow down to boot, so we might as well get started.
Colombian female. Controls 965 while it’s in the cruise. Passes it on to the approach frequency.
“American 965, descend and maintain flight level 240, report reaching.”
Colombian male. Controls and directs 965 onto the approach at Cali.
"Yes you can land runway 19, you can use runway 19. What is your altitude and DME from Cali?”
