"3 Greens" Air Crash Investigation - British Midlands 092
Project Overview
British Midlands 092 experienced a malfunction as it climbed out of London Heathrow on a Sunday night in January 1989. It was a malfunction to do with the left engine. However, the pilots misdiagnosed the problem and incorrectly shut down the right. Not realizing their mistake as they descended on low power settings. The remaining engine failed completely on approach to the runway. Leaving the aircraft powerless, descending and crashing short of the runway. Killing 47 of the 126 people on board.

Captain - 43 year old male. English. Professional yet relaxed tone even with the stress of an in-flight malfunction. Very experienced operator with 13,000 flight hours. Not as experienced on this exact type of aircraft, however. Which was a contributing factor in this accident.
ATC 1 - Support roles. Provides instructions to guide Midland 092 to an alternate airport.
“Midland 092, approach. Continue on your heading.”
“Prepare for crash landing. Prepare for crash landing.” (Urgent tone)
39 year old male. English. Becomes quite busy in the initial stages of the malfunction. Tone is composed but with a hint of urgency.
“London Center, Midland 092. We have an emergency situation. Looks like an engine fire.”
“Okay. Seems we’ve stabilized. I’ll start the engine failure and shutdown checklist. We’ve still got smoke.”
Flight Attendant - Middle aged female. Plays quite a significant role in this accident. As the passengers and flight attendants realized that something was still wrong even when the pilots didn’t. Hesitant but stressed tone.
ATC 2 - Support roles. Provides instructions to guide Midland 092 to an alternate airport.
Flight Attendant (PA): “All passengers please return to your seats and fasten your seat belts.”
ATC 1 (VHF Radio): “Midland 092 track direct to the Trent VOR. Cleared to climb flight level 350.” “V O R.” “three five zero.”
