Elmo is a washed up heroin addict who works at an Airport. Just like the rest of the cast from Sesame Street, Elmo is going through hard times. Elmo is a working puppet at a dead end job after his career ended. 

The voice actor for Elmo has to swear. It's a small role, but it triggers one of Kermit's flashbacks that explains everything.

Well, you know what’s going on with Elmo. Elmo loves heroin. (Elmo is explaining to Kermit what's been happening with him and the cast of Sesame Street.)
