This character has a constantly shifting voice, meaning, the pitch and attitude/mood of the voice should be frequently changing over and over. All of the changing emotions should follow the lines of anger, sadness, disgust or the like; all negative feelings. His voice should sound as if he's choking on blood, and have a substantial amount of reverb, echo and delay added to the voice as well. His voice should also convey a feeling of suffering, or some form of pain, while still sounding menacing.
