Aya receives an odd-colored apple from a mysterious old woman in a black hood, and eagerly shows it to Maria. Despite her assistant's warnings, Aya decides to take a bite from it and passes out. When she awakens, she discovers she's back in the Drevis Mansion, during the night of her mother's curse. Ogre reveals to her that her soul is trapped in a world created by Alfred's lingering feelings for her, and that if she wishes to wake up, she has to get to the source and make peace with him.

As Aya relives key moments from the event, the ghost of Jean Rooney occasionally pops up, and even attempts to drown her at one point. Eventually, Jean's ghost confronts Aya and asks her why she killed her, revealing that she aspired to become a doctor to help others just like Aya. Aya never gives her a response, though it's somewhat implied that she's shaken by what Jean said. Aya then continues her trip throughout the Drevis Mansion.

Additionally, Aya finds out some dark secrets about her mother as well. While coming across her Charm Encyclopedia, she finds an additional chapter that talks about increasing the odds to birth a boy, implying she wanted to have a son instead. Much later, she learns that Monika wasn't exactly sane during her youth, as she enjoyed writing letters to various killers and murderers to learn more about them. Monika was also part of a cult that was led by her grandfather, who's dream of creating "eternal beauty" led to the discovery of turning people into dolls. Monika was eager to continue his work until her grandmother told her that "true beauty" was leaving descendants for the future. Not wanting to disappoint either of them, she decided to compromise their separate wishes and give birth to her daughter, Aya, whom she planned to have continue her grandfather's legacy.

Eventually, she confronts her father's lingering will at the Secret Room, who attempts to kill Aya to turn her into a doll just like how the original attempted. Though Aya does flee, she briefly relents upon arriving at the Chapel, having decided to let Alfred kill her so that she could be reunited with him and Monika. However, upon hearing Maria's voice, she realizes she'd be leaving her all alone. Thus, she uses the Magic Water she obtained from Monika's ghost earlier and uses it on her father. She then tells him that she now understands why he wanted to turn her into a doll. However, she wants to continue to live, due to all that she experienced long ago, saying that she can't forget it no matter how hard she tries, claiming it must be part of the curse of the Drevis family. Before her soul leaves and returns to her body, Alfred's lingering will wish Aya to be happy with her life.

Back in her clinic, Aya wakes up to see Maria sitting nearby. The brunette scolds Aya for acting so hastily, before vowing to terminate the old woman who gave Aya that poisoned apple. Aya tells her there's no need, as she's certain that the old woman was actually a death god who sought to deliver karma to her. Admitting that they likely couldn't escape no matter how hard they try, Aya resigns herself to whatever fate has in store for her. She then asks if Maria would still stay by her side in spite of everything. Maria confirms that she's just as loyal to her as she was long ago, so Aya gives her assistant a smile.
